Step 1 - Application for Adoption

A $25.00 USD application fee is required per application which will be collected on Step 2 of this application form.

Thank you for having an interest in adopting a rescue dog from us. Although the application may seem lengthy, please know that it is necessary to ensure the safety and happiness of the dog. We also hope the application will help you reflect on circumstances that you may have forgotten to consider in deciding to adopt.

We ask that you keep in mind that many of these dogs come from foreign countries. As such, there is no way for us to know what they may have physically endured, how they have been treated, or how they’ve had to adapt to their environment. You must be willing to give them time to adjust to your household and to life here in the US/Canada. We offer no guarantee of behavior, age, health, life expectancy, breed, compatibility with other pets, or demeanor towards children because there are too many factors out of our control, and often no way to truly test each dog. If you are unwilling to take on this responsibility, then one of these dogs will not be the right choice for you.

We want to personally thank you, along with the long list of rescuers, clinics, transporters, fosters, flight companions, and everyone who has helped with the fundraising. It takes a coalition of people committed to saving lives and working together to bring these dogs home, and you are the final stop for these tattered paws with golden hearts.

Prior to submitting an adoption application, please consider your personal current circumstances and upcoming events, (for instance: vacations, long term leaves of absence, relocations, financial changes or a change in family dynamics) as this may interfere with a proper introduction and the amount of time needed to help your new dog adjust to your home and routine. 27. In nearly every circumstance, we require that all other dogs residing in the home be spayed or neutered. In limited situations, such as for medical reasons, we may allow for the adoption of an unaltered dog. In these circumstances, the dog must not be in the company of an animal of the opposite sex who has not been spayed or neutered. Do you have any dogs in your household that are not spayed or neutered?

We will reference check with your veterinarian/clinic, as such, we will need you to provide authority for your veterinarian/clinic to speak with us. Do you agree?

30. Does anyone in your household have a fear of dogs? If yes, please explain.

31. Does anyone in your household have allergies to animals? If so, please explain how they manage their allergies?

32. Have you ever released a pet to an animal shelter, rehomed, or given away an animal? If yes, please explain the circumstances.

33. This dog must not be left unattended outside, this includes in a yard where he or she would be easily accessible to strangers or animals, tied up outside a public location such as a store, or on a sidewalk. He or she may not ride in the back of a truck or other open vehicle without a locking canopy or similar secure covering. Dogs are often being stolen and used as bait dogs, purposely injured in order to obtain medication, sold on sites for money and for many other unbelievable reasons. Are you willing to comply with this?

34. On average, how many hours per day will your dog spend alone? How will you prepare him or her for this (e.g., provide toys, treats such as a stuffed Kong, radio or other sounds, crated, use of anxiety vest)?

35. Many of these dogs have never been given formal training. Many dogs experience separation anxiety, barking problems, leash pulling, jumping, dominance issues, potty accidents, ect., Do you truly feel you are prepared to overcome these issues through training? Are you willing to personally train this dog? Are you willing to hire a licensed dog trainer to help if needed?

36. While we do our best to screen for and treat medical issues in Turkey, dogs can always arrive with issues that were undetectable or common, including dental issues, ear infections, parasites, hot spots, ect. If your dog were to have expensive medical needs, would you financially be able to assist them?

37. Have you ever been convicted of any animal-based violation, infraction, offense, or crime in any jurisdiction, including but not limited to neglect, cruelty, abandonment, etc.? If so, please identify the jurisdiction, year, charge, case number, and outcome.

38. Have you ever had a pet euthanized? If yes, please explain the reason that this was necessary.

39. Can you afford veterinary care, grooming, emergency expenses, supplies and quality food for the lifetime of this dog?

40. What type of food do you plan on feeding this dog?

41. Can you think of any situations that might cause you to return this dog?

42. If your adoption application were to be approved, would you agree to return this dog to TPGHR immediately if you were no longer able to keep or provide care for this dog? If you were unable to keep your dog, we are willing to consider other homes that you might suggest; however, the final decision would be ours.

43. Are you willing to let a volunteer visit your home by appointment to conduct a home visit and provide you with some recommendations if needed? TPGHR reserves right, in its sole discretion to determine whether this home visit would be conducted virtually or in person.

44. It is very important to the rescuers in these other countries to occasionally receive an update on how this dog is adjusting to life in the U.S. They go to great lengths to save them. This is a small thing to ask. It also gives them a moment of happiness as a reward for all that they do to save these dogs. Assuming your application is accepted, will you be willing to keep this communication going either through us or directly with them?

45. Assuming your application is accepted, will you agree to accept responsibility and ownership of the rescued dog at your own risk and agree to release Tattered Paws and Golden Hearts Rescue, all its directors, officers, volunteers, agents, foster homes and representatives, and all others involved in rescuing and transporting the dog from overseas, from any and all liability arising out of possession and ownership of your rescued dog?

46. Assuming your application is accepted, will you agree to assume all total financial responsibility for your rescued dog from the time he or she is delivered to you until and including the date he or she dies, meaning that Tattered Paws and Golden Hearts Rescue, and all its directors, officers, volunteers, agents, foster homes and representatives, and all others involved in rescuing and transporting the dog from overseas, will not be held responsible for any damages or expenses (veterinary or other)?

47. Assuming your application is accepted, will you also agree to notify TPGHR of each and every change of contact information (address, phone number, email address)?

Disclaimer & Release

I have read the above information carefully and have filled out the questions honestly and to the best of my ability. I understand that omission of information and/or failure to answer all questions can result in my application being denied. I also understand that the adoption decision is dependent upon many factors, including but not limited to compatibility of the family and home to the individual animal. TPGHR reserves the right to deny an application to anyone for any reason. TPGHR also reserves the right to not disclose the reason for rejecting an application.

I further understand that this is merely an application to adopt. Each adopter is required to sign an adoption agreement prior to the transfer of ownership of any dog.
